mentions pan, perhaps this is fixable.

David was not trying to say "this is not possible in MIDI".  He was trying to say "this is not possible in Lilypond."  Lilypond does not, in general, provide access to arbitrary sysex features in MIDI.

There are certainly tools that allow you post-process a MIDI file to set the panning positions.
