Or perhaps dropped.. the handle to the object being invalid.

No.

No, but GUI interaction / timing issues can change how the stack
or heap looks, and thus the nature of a crash.  Then there
can be uninitialized heap/stack data.  Computers can be suprisingly
non-determinsitic.  ;-)
