If you have a middle mouse button.  Some of us have to hit both buttons
to emulate the middle one, and that can be both inconvenient and
unreliable.

At one point I thought I read that ctrl-leftclick shouldn't pop up the
save dialog, but it seems to when no article is currently selected.

Anyway, I'd vote for no save dialog for binaries when you left-click on
one.  I always have to cancel because I need to include other articles.
